Dr. Bajaj,


You performed a provocative discogram on my lower back (L3/L4, L4/L5,
L5/S1) on Monday December 3rd. We were supposed to meet on December 18th to discuss the results, but unfortunately you called in sick that day. On January 1, I switch from Kaiser HMO to Blue Cross PPO insurance, and I am currently out of town until December 29. Therefore it appears I will not be able to meet with you. I tried to schedule an appointment with you for Monday December 31, but I was told you were not available that day. I have arranged to meet with Dr. Hilliard, but it seems important to discuss the test results with you since you actually performed the discogram.

In order for me to best understand what was learned from the procedure
so that I can convey that to my new doctor(s), I am asking you to please provide me as much information as possible regarding the discogram (i.e.: the results, conclusions, recommendations, next steps, etc.) Also, if you know of any outstanding doctor(s) who you would recommend for my situation, please let me know who they are. With my new PPO insurance, I will be able to self-refer to the specialist of my choice. If I am going to require a procedure to fix this chronic lower back pain, then I want to get the absolute best care available.
