You performed a provocative discogram on my lower back (L3/L4, L4/L5, L5/S1) on 12/3. I switch insurance to Blue Cross PPO on Jan. 1, and I am currently out of town until 12/29. I have arranged to meet with Dr. Hilliard on 12/31, but it seems important to discuss the test results with you since you actually performed the discogram.


In order to best understand what was learned from the procedure so that I can convey that to my new doctor, I am asking you to provide me as much information as possible regarding the discogram (results, conclusions, recommendations, next steps, etc.) Also, if you know of any outstanding doctors who you would recommend for my situation, please let me know who they are. With my new PPO insurance, I will be able to self-refer to the specialist of my choice.
